Therapeutic Supervised Visitation (TSV) provides therapeutic supervised and progressive visitation between birth parents and their children with the goal of family reunification.
The Care Coordinator provides assistance with the supervision of visits, outreach, supportive counseling, case management, transportation and psycho educational services to consumers. Maintains contact with DCP&P and coordinates with family, court and other treatment providers/significant others. Develops effective intervention strategies for children with emotional and behavioral problems, trains and motivates parents to provide effective care. Documents interventions used during visits and consumers responses to interventions. Has thorough knowledge on community resources, child development, family dynamics, and psychological impact of abuse in trauma. Is able to support a family’s spiritual and cultural background and practices. Responds to client crisis with prescribed procedures for de-escalation, crisis management, and behavior management. Demonstrates a trauma-informed response to clients based on Safe Crisis Management guidelines. Communicates observations both verbally and in the client’s electronic health record using prescribed formats and terminology. Participates in and accepts supervision and feedback as scheduled with supervisor. Follows agency policies and procedures regarding work practices and expectations. Excellent navigation skills and proficiency in using navigation applications to find locations. Time management and organizational skills to keep track of visits and stay on schedule to meet program guidelines. Ensures the children have good hygiene and changes diapers and feeds infants/toddlers as necessary.
This position is full-time, with 2 potential late nights being required, as we work with school-aged children and are not always able to conduct visits during normal business hours. A company vehicle is used for transport, and all devices used to complete documentation and reach out to consumers and providers are provided by the agency.

Minimum of Bachelor’s Degree in mental health field, social work or related field. Case management experience required.
